Swimming is muscular endurance almost to the exclusion of strength. You move a light resistance thousands of times per session, which builds fatigue resistance and does very little for maximum force, muscle mass or bone. The consequence is specific and predictable: a swimmer’s shoulder is repeatedly driven overhead by muscles that are enduring but not stable, and the small muscles that hold the joint together are never trained. Swimmer’s shoulder is the commonest injury in the sport, and it is largely preventable on land.

Endurance and Strength Are Different Things

A swimmer with excellent muscular endurance and no strength work is a swimmer with a shoulder on a timer.
Why Shoulders Break in Swimmers
Freestyle drives the arm overhead somewhere in the region of a thousand times an hour. Each rotation asks the shoulder blade to move correctly on the ribcage and the rotator cuff to keep the ball centred in the socket.
Swimming trains the big movers — lats, chest — and neglects the small stabilisers. It also shortens the front of the chest and leaves the upper back long and weak. The joint drifts forward. The space the tendons pass through narrows. And then one Tuesday it hurts.
Three things accelerate this: paddles, which amplify a dropped elbow directly into the joint; volume added too fast; and a poor catch, where the elbow drops and the arm pulls from a compromised position.
The Work That Protects It
- External rotations. Light band or dumbbell, elbow at the side. Two sets of fifteen. Boring. Essential.
- Band pull-aparts. Two sets of fifteen, light, before every swim if you can.
- Rows. Any variety. The upper back is the counterweight to everything swimming does.
- Scapular control work. Wall slides, prone Y and T raises. Slow.
- Chest and lat mobility. Not before a swim — afterwards, or on a separate day.
- Something that loads bone. Squats, hinges, carries. Swimming provides none.
Two sessions a week, thirty minutes each. That is the whole prescription for a swimmer whose main sport is swimming.
What to Avoid
- Heavy overhead pressing, at least early on. Your shoulder already works overhead for hours a week.
- High-volume bench pressing. It tightens a chest swimming already tightens.
- Behind-the-neck anything.
- Paddles. Especially large ones, especially before a coach has approved your catch.
- Adding swim volume and strength volume in the same week. Progress one.
- Training through shoulder pain. It does not respond to pushing through.
If Your Shoulder Already Hurts
Stop. Not “ease off”. Stop the aggravating work.
Swimmer’s shoulder is an overuse injury. Remove the paddles, cut the volume, and see a physiotherapist. Do not attempt to strengthen through an impinged shoulder — loading a joint that is already pinching makes it worse, and the internet is full of exercises that will do exactly that.
Do not swim butterfly. Do not do heavy overhead work. Swim freestyle only if it is pain-free, and stop the moment it is not.
Breaststroke knee is the same story with a different joint. Stop the stroke and get it assessed.
Safety
If you have never lifted, get taught. A few sessions with a qualified coach prevents years of avoidable problems.
See a doctor before starting if you have a heart condition, high blood pressure, are pregnant, take medication affecting heart rate or blood pressure, or have not exercised in years. Never hold your breath through a heavy repetition — straining raises blood pressure sharply. Breathe out on the effort.
In the water: swim where a lifeguard is on duty, never alone, and never use breath-holding or hypoxic sets. Prolonged or repeated breath-holding, especially after hyperventilating, causes shallow-water blackout — a silent, warningless loss of consciousness that has drowned strong swimmers in supervised pools.
And drink. You sweat in the water and never notice.
The Uncomfortable Conclusion
Swimming is superb for your heart, your joints, your longevity and your sanity. It is not sufficient exercise on its own, and a swimming school that tells you otherwise is selling rather than coaching.
Two half-hours a week on land protects the shoulder that lets you swim, defends the bone density that swimming ignores, and preserves the muscle mass that decides how you move at eighty.
Meanwhile the highest-yield thing available in the pool is still technique. A dropped elbow cannot be strengthened around — it has to be fixed.
